System Administrator access is required to manage hazards.

1. Adding a new hazard
First navigate to the Incident module to see a list of issue types.
Select Hazard as shown below.
1.1. Fill in the hazard form with all the required details
This will open a new hazard form with the fields shown below. Fill in the fields required and submit once ready. For a full list of the fields and descriptions of each field see the table below.
2. Definitions of hazard fields
| Supervisor | Select the supervisor responsible for the hazard. |
| Is Improvement Opportunity | Select if the hazard provides an improvement opportunity. |
| Report Type |
Select the report type. For example: Hazard, other. |
| Source of Hazard |
Select the source of the hazard. For example: plant and equipment, physical environment. |
| Identified Potential/ Actual Hazard | Select all identified potential and actual hazard(s). |
| Details | Enter a full description of the hazard. |
| Immediate Action | Enter any immediate action that has been taken because of the hazard being identified. |
