1. Form design

Form fields create forms, accessible from the left-hand library when creating or editing. The form is built in the right-hand layout area. Instructional videos are available for most features.

Click a field type to add to bottom of form.
Drag the field type to insert elsewhere.

Form fields	Description
Asset Integration	Displays asset fields if selected in Form Admin. Respondent selects asset; other fields auto-populate.
Text	Labelled box for basic information. Supports attachments and actions.
Paragraph	Larger text box. Supports attachments and actions.
Checkbox	For multiple answers.
Radio Button	For single, short answers (2-3 words).
Time	For specific time recording.
Date	For date input (dd/mm/yyyy or calendar).
Dropdown	For single, longer answers (4+ words). Recommended when options are too long for radio buttons.
Number	Numeric input only.
Signature	For electronic signatures.
Table	Creates flexible tables with various field types. Respondents can add/delete rows (max 30). Supports column auto-totals for number fields. Requires Display column total to be ticked in configurable attributes. Allows row auto-totals and field multiplication Sum option: Adds all number fields in a row. Product option: Multiplies all number fields in a row. Separate videos available for introduction and table sums.
Section Break	Divides form into sections with headings.
Info	Adds notes, instructions, or hyperlinks. Can include inline photos and actions. Example: Use in generic Toolbox Talk form with hyperlink to updateable agenda items.
Data Source	Pulls data from other Lucidity modules. Types: Company, Employee, Person, Project, Asset. Supports auto-population of additional data. For long lists, include instructions: "start typing to search". Consider security for public forms: Risk of exposing employee names or other sensitive data to public.
Geo Location	Records respondent's location (latitude/longitude). Displays in browser and Lucidity App.
Approval	Adds workflow approval process. Can set fixed or variable approvers. Includes automatic signature box.
